If the portal asks for a licence or credential type, choose the one that matches the CDPH phlebotomy certificate (LPT, CPT I, or CPT II) — these systems list every credential the agency issues, not just phlebotomy.
Search by name or number
Start broad — last name only. If you have the credential number, search by that alone for an exact match. Middle initials, hyphenated surnames, and recent name changes are the most common reasons a search returns nothing.
Read and save the record
Confirm the name matches, the status reads active or current, and the expiration date has not passed. Employers: save a dated screenshot or PDF for the personnel file.

Active / Current
The credential is valid and the phlebotomist is cleared to work in California. Still check the expiration date — "active today" is not "active at renewal time."
Expired / Delinquent / Inactive
The credential is no longer current. Treat the phlebotomist as not cleared until the agency renews or reinstates it — in a state that requires the credential, a lapse is a compliance problem for the employer as well as the individual.
No results
Usually a name mismatch — retry with the last name alone, or with the credential number. If the record genuinely does not exist, ask the candidate for the credential number before drawing any conclusion, then contact the agency.

What California Employers Check
A credential check is one part of what a California hospital, reference laboratory, clinic, or plasma centre confirms before a phlebotomist starts. In practice they verify:
- The California credential is active. This is the one that authorises the draw. An expired credential means the person may not legally work as a phlebotomist here, regardless of what else they hold.
- The national certification is current, at the source. Not a photocopied card — the certifying body's registry, which shows the real status and expiration date.
- Training and documented draws. Employers ask for evidence of a completed phlebotomy programme and a documented number of successful live venipunctures and capillary sticks; the certifying bodies set floors ranging from 30 venipunctures for NHA eligibility to 100 for certain ASCP routes.
- Current BLS/CPR, immunisations, background check and drug screen. Standard across the industry, and usually re-checked at intervals.
Phlebotomists: run the same checks on yourself once a year. It is the fastest way to confirm your record shows exactly what a hiring manager will see, and to catch a renewal that never posted.

How do I verify a phlebotomy license in California?
Use the CDPH LFS Personnel Licensing Search at cdph.ca.gov, the California Department of Public Health, Laboratory Field Services's official public search. Look the phlebotomist up by name or credential number; the result shows whether the credential is active and when it expires. The search is free and needs no account.
What does a California phlebotomy record show?
The official record shows the holder's name, the credential type and number, its current status, and its expiration date. In California the credential you are confirming is the CDPH phlebotomy certificate (LPT, CPT I, or CPT II).
How often does a California phlebotomy credential have to be renewed?
Annual. Beginning January 1, 2026, all LFS personnel licenses and certificates moved to a one-year renewal period (certificates may still display a two-year expiration date during the IT transition, but renewal is required after one year). Continuing education: LPT, CPT I, and CPT II holders must complete 3 contact hours of continuing education from a Department-approved accrediting agency or accredited academic institution within the last year; renewal of an expired certificate requires 3 contact hours within the last 12 months. Renewal is filed through the LFS online system (PERL) with CE certificates uploaded. Renewal of the underlying national certification is optional and separate.
